SHORT TERM MISSION TRIP PARTICIPATION AGREEMENT
Thank you for your interest in joining one of our short-term mission trips! We are called to be the church and spread the great news of the gospel of Jesus Christ. As a church, we believe our role is to engage in telling others about Jesus both locally and internationally. While many churches send their funds overseas, we believe partnership involves more than just sending money—we give by sending ourselves to partner with our fellow brothers and sisters in Christ serving abroad.
OBJECTIVE: To participate as a team member on a short-term mission trip with ONE Church and have the opportunity to see and participate in missions firsthand. By teaming up with veteran missionaries, mission teams can leave a lasting impact on foreign works as well as allow God to develop their understanding and burden for missions. Because our mission’s program is local church based, participants may be assured that they are working within the model left for us by Christ Himself.
REQUIREMENTS: Each short-term mission trip participant must be in good health and in good standing with the church and community. To qualify, each participant must:
• Submit a signed Mission Trip Participation Agreement (included in this form)
• Submit a signed Mission Trip Personal Covenant (included in this form)
• Submit a signed Mission Trip Risk Acknowledgement (included in this form)
• Submit a Signed Medical Release Form (For the 2025 trip, the Adult Medical Release and the Under 18 Medical Release forms must be filled out by hand. Please contact One Church or Brian Pritchard to obtain a copy.)
• Obtain a U.S. passport prior to departure.
• Obtain a visa (If applicable).
• Complete a criminal background check coordinated through ONE Church
• Attend pre-trip meetings.
• Participate in fundraising efforts.
• Stay on schedule with pre-payment plan. The trip Team Leader or Mission Coordinator may check in with you to ensure you are reaching funding goals.
ITINERARY: ONE Church will establish the trip itinerary, including departure and return dates, and purchase international air travel tickets for each group or individual on the mission. Individuals may not purchase their own international air tickets. Requests for individual changes to the established international travel itinerary are strongly discouraged.
PASSPORTS: All individuals traveling outside the United States of America MUST have a valid passport. First time passport applicants should apply as soon as possible as response times vary depending on the time of the year and region. One should expect to plan 120 days processing time for a new passport. Many Post Offices have facilities to assist in obtaining passports. In Beckham County, all passports are handled by the courthouse in Sayre. All individuals are responsible for the cost of their own passport.
At the time of visa application, all passports must have at least six months remaining before expiration.
VISAS: All individuals are responsible to apply for their own visas.
CANCELLATION POLICY: All individuals will receive a full refund of the deposit and contributions up to the date of securing airfare. Once airfare has been purchased, only the amount over the cost of airfare will be returned. Because airlines do not issue refunds, the church cannot recoup the cost of airfare. Therefore, you will be responsible to pay the full airfare expense even if you cancel once tickets have been purchased.

SHORT-TERM MISSION TRIP PERSONAL COVENANT
As a member of a mission team sponsored by ONE Church, I agree to the following:
• I will go as a servant-disciple of Jesus Christ and will adopt that attitude when dealing with my fellow team members.
• I will remember that I am an unknown Christian from another part of the world and will be watched very closely.
• I will not take this responsibility of setting a good example lightly.
• I will accept the leadership authority of the Team Leader and abide by his/her decisions as they concern the mission trip.
• I will attend all team meetings possible, both prior to departure and during the mission trip.
• I will refrain from all gossiping, complaining, and obscene or insensitive humor. We must all try to get along.
• I understand that travel, especially to remote locations, can be difficult, and I promise to adopt a flexible attitude and be supportive as plans may need to be changed.
• I will abstain from the use of any illegal drugs and/or prohibited activity while on this trip.
• I will act as a servant-disciple of the local pastor or mission organization.
• I will respect the advice I am given concerning attire, eating and drinking, and other such traditions which will help me to assimilate into the local community. The host/church has given you their very best of accommodations, please be courteous.
• I agree that in the event my conduct is considered so unsatisfactory that it jeopardizes the success of the trip that my services in connection with this mission trip shall end and I shall return home immediately at my own expense.
• I understand and agree that I will be held financially liable for any property which is broken, damaged, or destroyed due to my own negligence or by any deliberate act on my part, including but not limited to: graffiti, vandalism, and mishandling or misuse of tools, vehicles and equipment.
